Forma Health was founded by 22-Grand Slam winner Rafael Nadal and Dr. Ángel Ruiz-Cotorro, Forma leverages AI to prevent injuries in tennis players across all levels.
We partnered with Rafael Nadal and his longtime doctor, Dr. Ángel Ruiz Cotorro, to help incubate and launch Forma—the world’s first holistic, digital injury prevention and recovery platform designed specifically for tennis. We worked closely with them to shape the original concept, define the product strategy, and lead design and development across the app, wearable integration, and brand.
Tennis injuries are on the rise, especially among young and developing players. The goal of Forma was to make elite-level sports medicine—traditionally reserved for top professionals—accessible to any player, anywhere. We helped build a platform that blends wearable data, expert medical knowledge, and sport-specific content into a single, user-friendly experience.
Forma offers a wide range of features including the Forma Score, a proprietary readiness metric; daily recovery check-ins; mobile biomechanics analysis via camera; and customized prevention routines. Educational modules cover everything from injury recovery to nutrition and sleep, while a pain and injury reporting tool helps players and coaches take early action. Through a strategic partnership with Polar, Forma also includes a connected wearable that supports fitness tracking and recovery insights.
Several top-ranked international players and leading tennis academies now rely on Forma to support performance and reduce injury risk. Backed by one of Europe’s largest medical groups, the platform has gained strong traction in the tennis world and is currently expanding into adjacent sports.
With a public launch planned for 2025 and over €1M raised, Forma is redefining proactive athlete care—bridging the gap between medicine and performance with intuitive, data-driven tools.
